**DISCOVERY CALL ** - 20 min call where we want to find out about your injury and what your goals are. We also want to see if you will be a good fit for our clinic
DISCOVERY SESSION FREE 30 min (in office) - FIRST TIME CLIENTS ONLY -We will go over a detailed History of why you are here and see if we are a right fit you.
INITIAL EVALUATION & EXAM - -Detailed movement exam, ROM, Active testing, treatment and therapy procedures. - Explain our next steps in the plan of care process for you.
PLAN of CARE - POC - Packages - Payment for treatment packages must be made upfront, either with the discount or after the initial evaluation. - No refunds for treatments already received, except for non-negligent treatment. - Exiting a plan early cancels the bulk discount; the patient will be charged at the current rate per treatment.
BILLING Your credit card will not be charged until after your appointment. FSA & HSA cards accepted. We are “out of network” with all medical insurance companies, but are happy to provide you with detailed receipts/superbills should you choose to file your own insurance.
Your appointment time is reserved just for you. To better serve our patients and ensure that they have a fair opportunity to have an appointment as promptly as possible, we require a minimum 24-hour notice to cancel or reschedule appointments.
CANCELATION POLICY: Patients who fail to show or cancel/reschedule an appointment and have not contacted our office with at least 24 hours notice will be considered a “No Show” and charged $FULL RATE for each missed appointment. The fee is charged to the patient, using the card on file.
As a courtesy, our system will send an appointment reminder 24 hours before by email and 2 hours before by text message, unless you have opted out of these services. Then it becomes your responsibility to make sure you are on time to your scheduled appt.
